This form must be completed by students applying for Pre-Completion Optional Practical Training, Post-Completion Optional Practical Training, and 17-Month Extension of Post-Completion Optional Practical Training.
It is recommended that you download and print form I-765: http://www.uscis.gov/files/form/i-765.pdf and have the printed form in front of you prior to viewing the webcast, so that you can more easily follow the webcast instructions. The form can be saved on your computer, so the final version that you will submit as part of your OPT application to US Citizenship and Immigration Services should be filled out on-line.
