If an individual is not engaged in the support services indicated above, the University will supply up to 2 weeks of NRT treatment.

Requires your doctor to obtain notification/prior authorization. Requires notice from your doctor that you are receiving tobacco cessation counseling. Requires prescription from your doctor even if products sold over the counter. Must be 18 or older. Must fill the prescription at a network pharmacy.
Determination of payable benefits made when actual claim for services is received. Benefit payment shall be subject to all deductible, co-payment, co-insurance, exclusions and limitations, or any other provisions of the policy in effect at the time of service.
