Health Insurance Waiver Request

This E-Form can be used by students to submit a request to have their International Health Insurance Fee waived.

There are several different conditions under which a student may request the waiver:

  1. Student is a Graduate Student with an Academic Assignment as a Teaching, Graduate or Research Assistant;
  2. Currently studying in home country;
  3. On CPT with employer health insurance;
  4. Has a visa type other than F-1 or J-1;
  5. Has private insurance.

If a waiver is requested for either #3 or #5, you will need to upload the following information:

  • Copy of your current insurance policy (full version in English), specifying benefits and coverage;
  • Front and back sides of your current insurance card (showing name and coverage dates) OR certificate of enrollment.  This may be done in a single file, or separate files for each side of the card.

If you need to upload documents, they must be in PDF or image (JPG, JPEG, GIFF, PNG) format.

Here are the instructions for submitting the Health Insurance Waiver e-form:

  1. Login to Sunapsis.  Click here for instructions.
  2. After logging in, click on ‘Insurance and Finances’ in the shaded box on the left side of the screen, then select ‘Health Insurance Waiver’:Health Insurance Waiver Screenshot

  4. Information about the health insurance waiver request must be entered, including (all fields are required):
    1. Visa Type;
    2. Term (Fall or Spring);
    3. Year;
    4. Funding / Adjustment Type.
  5. If ‘In home country’ is selected, you must upload one of the following documents:
    1. Passport page with stamp showing date on which you entered your home country, or

  6. If ‘CPT with Employer Insurance’ is selected, you must upload the following documentation:
    1. Copy of your insurance policy;
    2. Front and back sides of your current insurance card OR certificate of enrollment.CPT Employer Insurance Information Screenshot
  7. If ‘Private Insurance’ is selected AND you have previously been granted a waiver for this reason, you will need to upload the front and back sides of your current insurance card OR certificate of enrollment:Private Insurance Information Screenshot
  8. If ‘Private Insurance’ is selected AND this is the first time you are requesting a waiver for this reason, you will need to upload the following information:
    1. Copy of your insurance policy;
    2. Front and back sides of your current insurance card OR certificate of enrollment.Private Insurance-No Option Selected
  9. If ‘Other’ is selected, a brief explanation is required:Insurance Waiver Other Funding Type Selected
  10. Read the statement in the confirmation box CAREFULLY.  You must click on the box to signify you have read and understand the statement. When complete, click on the ‘Submit’ button at the bottom of the form.Health Insurance Waiver
  11. The top of the form will be displayed, with a message stating the form has been submitted and is under review.  You may select another E-Form to process, or log out of Sunapsis by clicking on ‘Logout of iSSS BU’ in the shaded box:Health Insurance Waiver Form Submitted Screenshot
  12. Your request will be processed within 3-5 business days. You will receive a notification whether it is approved or denied. If approved, your Student Account will be credited.
  13. If you want to cancel the E-Form, scroll to the bottom and click the ‘Cancel’ button:Health Insurance Waiver
