Process to Register with SSD

- Student completes the online “Link to Student Self-Disclosure Form” uploading the appropriate documentation to the form. Please review the documentation requirements link.
- You will receive an email from an SSD staff member.
- Student meets with an SSD staff person to review documentation, history of accommodations and the functional impact of the disability.
- SSD will determine and authorize equal access accommodations.
