What to do Before Attending Orientation

Students must complete the following forms prior to attending orientation!
Not completing the following prior to the start of the summer program will delay your ability to register for classes. Paper copies of this information will be sent to accepted students.

1. Read the following:
   • Welcome Information for New EOP Freshmen
   • EOP Orientation & Parent/Guardian Meeting Details
2. Reserve your Orientation date and Bus Seat (if applicable) using the
   • Orientation and Bus Reservation Form
3. Completed the Following EOP Forms:
   • EOP Student Parental Consent Form & Emergency Contact and Authorization for Release Form
4. Complete the following USDA form
   • EOP Summer Meal Form 2024
5. Complete and upload all Health forms into the Patient Portal
   • Immunization Form (Doctor Signature Required)
   • Meningococcal Vaccination Form
   • Mandatory Medical/health history/tuberculosis questionnaire:
   • Under 18 Consent Form (if applicable)

6. Upload your ID Picture. At this time, please use your desktop to upload your photo rather than your cell phone. If you have issues uploading your photo, please email your photo to idcard@binghamton.edu
7. Complete your FAFSA.
8. Complete the BEP Multi-Ethnic Dinner Recipes
9. Confirm your Enrollment
   • Once students have been registered for classes, they will receive a message from Binghamton University asking them to confirm their enrollment. Confirmation allows the students to remain enrolled in courses that they have been registered
