ACCESSING YOUR HEALTH INSURANCE CARDS
To obtain your Member ID, employees can call Member Services for each corresponding insurance plan. Employees will need to provide their Social Security Number and other personal information to validate identity. Member Services will provide the employee with their Member ID which will allow them to register on the carrier’s website/apps, access their electronic ID card and other member materials. The Member ID will be the same for all covered dependents.

Anthem Blue Cross
- Visit the Anthem Blue Cross Register Now page, select “Member ID” and enter required personal information. (You may enter your SSN as the member ID) OR
- Download the Sydney Health app to access your electronic benefits. (You may enter your SSN as the member ID)
- Employees can contact Member Services at 800-342-9816 and provide their social security number and other identifying information to obtain their Member ID
Prescription (Express Scripts) – For Anthem subscribers only

- Visit the Express Scripts Register Now page and enter the personal data information required as well as the last 4 digits of your Social Security Number OR
- Download the Express Scripts app to access your electronic benefits.
- Employees can also contact Member Services at 800-251-7690 and provide their social security number and other identifying information to obtain their Member ID

Regular Employees and Postdocs will receive 2 ID cards; 1 for Anthem Blue Cross and 1 for Express Scripts. Ensure you are only presenting your Express Scripts card at a pharmacy.
Graduate Student Employees will receive 1 ID card. This will allow access to the Anthem Blue Cross benefits and the Express Scripts benefits.
CDPHP
- Visit the CDPHP Register Now page. Employees will need their Member ID to create an account
- Employees can contact Member Services at 800-777-2273 and provide their social security number and other identifying information to obtain their Member ID
MVP
- Visit the MVP Register Now page. Employees will need their Member ID to create an account
- Employees can contact Member Services at 888-687-6277 and provide their social security number and other identifying information to obtain their Member ID
Delta Dental (Note: Delta Dental does not mail physical cards)
- Visit the Delta Dental Register Now page or download the Delta Dental app
- Registration can be completed using your Social Security Number or Member ID
- Employees can contact Member Services at 800-932-0783 and provide their social security number and other identifying information to obtain their Member ID
