EAP offers confidential consultations to individual employees as well as supervising employees

Individual employees may fill out the form to coordinate an Assessment and Referral consultation. During this consultation session, an EAP Coordinator will work with you in identifying resources to help navigate work-life challenges. Supervising employees may fill out the form to coordinate a Supervisory Consultation. During this consultation session, EAP Coordinators can assist supervisors in developing an appropriate plan to assess departmental or individual issues and recommend referral resources where appropriate.
